- Acceptance of Terms: By enrolling in our de-addiction center, you agree to abide by these terms and conditions, as well as any rules and regulations established by the center.
- Eligibility: Our services are available to individuals who are battling addiction and are committed to recovery. Admission is subject to an initial assessment and approval by our medical team.
- Confidentiality: All patient information will be kept confidential in accordance with our Privacy Policy. However, information may be disclosed to relevant authorities if required by law.
- Treatment Plan: A customized treatment plan will be developed for each patient. Adherence to this plan is crucial for successful recovery. Patients must attend all scheduled sessions and participate fully in the program.
- Rules and Conduct: Patients are required to follow all rules and regulations set by the center. Any form of violence, substance use, or disruptive behavior will result in immediate termination of services.
- Payment and Fees: Payment for services must be made in accordance with the payment plan agreed upon during admission. Failure to make timely payments may result in suspension or termination of services.
- Medical Care: The center provides medical care as part of the treatment plan. However, patients are responsible for informing the staff of any pre-existing medical conditions or allergies.
- Termination of Services: The center reserves the right to terminate services if the patient violates any terms of this agreement or if the treatment team determines that the program is not beneficial to the patient.
- Limitation of Liability: The center is not liable for any damages or injuries that occur as a result of non-compliance with the treatment plan or center rules.
- Changes to Terms: The center reserves the right to modify these terms and conditions at any time. Patients will be notified of any changes.
